1. Permission to Participate

I am the parent or legal guardian of the above-named minor (“Participant”). I give permission for my child to participate in the above-described ministry activity and related travel, including transportation arranged or supervised by The Creek Church (“Church”).

2. Assumption of Risk

I understand that participation in travel and ministry activities may involve inherent risks, including but not limited to transportation risks, physical activity risks, weather conditions, illness, accidents, and the actions of third parties. I voluntarily assume all such risks on behalf of myself and my child, whether known or unknown, foreseeable or unforeseeable.

3. Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge The Creek Church and its pastors, employees, volunteers, leaders, members, and agents from any and all claims, demands, causes of action, or liability arising out of or related to my child’s participation in the activity and travel, except to the extent caused by the gross negligence or willful misconduct of the Church.

4. Idemnification

I agree to indemnify and hold harmless the Church from any claims, damages, or expenses (including attorneys’ fees) arising from my child’s participation in the activity, except to the extent caused by the gross negligence or willful misconduct of the Church.

5. Transportation Consent

I authorize my child to travel by vehicle, van, bus, airplane, or other transportation arranged or approved by the Church and understand that volunteer drivers may be used.

This Agreement shall be governed by the laws of the Commonwealth of Kentucky.
